Media Release

This form is to be completed by a parent or guardian for a child under the age of 18 years. This form is for images, video, and recordings to be used solely for Southview Church ministry purposes on approved, live streaming, social media sites, Southview Church website, and internal print materials. PLEASE READ THE FOLLOWING CAREFULLY. I hereby grant Southview Church an absolute and irrevocable right and permission, in respect of any photographs, motion pictures, recording or any other media whatsoever (“Images”) which are taken by an approved Southview Church person or staff of my child or in which my child may be included with others, to use, re-use, publish and re-republish the Images in whole or in part, individually or in conjunction with other Images, and in conjunction with any printed or electronic media now or hereafter know, and for any legitimate purpose whatsoever. I waive any right to inspect or approve the Images or any finished product using the same of my child. I waive and release any right or claim I or my child may have to receive an compensation or reimbursement in regard to any of the foregoing. This consent, agreement, waiver, and release applies regardless of whether my child was involved in the creation or production of any such Images or finished product and regardless of whether any such right or interest arises by virtue of statute or otherwise.

I commit to pursue a growing and healthy relationship with Jesus, asking good questions along the way, and surrounding myself with other faith-filled believers. I will seek to live and act in the ways Christ modelled and God the Father calls us to.

I to commit to serving all five days of camp. If something comes up that would change this, I will let Southview Kids staff know as soon as possible. I understand that this might result in staff needing to change the type volunteer role I am in. If a last-minute emergency or illness arises, I will call 403-873-5240.
